Being that I do not sit at the computer and check email all day it is best to call or text for same day appointments. All I need is a 30 minute heads up.

It's best to use the form if you're interested in setting up an appointment for a future date. Also if you fill out the form to request an appointment you will still have to call the above number on the day of your appointment at least an hour ahead of time to confirm.
